the'time of installation of. a meter and ser:vice:connection:on .theCitls nearest main, and

4. Any breach or violation of the terms and conditions established by this

witbinthirty days after written notice giVen toOwnerortheir successors in interest by City
'lfFtheeventof such termination, City may disconnect the line without any further liability or

5 Owner hereby fU,rthe( agrees to pacticipate in the formation of a special
assessmentdistricf,pursLJar'1!:to tneprovisions ofthe "MunicipallmprovementAct of 1913",
or the "Improvenient Act of1911 ", for the construction:and installation of apermanentwater
systeni. ~ Owner fucther acknowledges that his ~property will be assessed for its
proportionate share of the costs, of the work of improVement. Owner waives any rights, at
this time, to protestor object totheJormation of the assessment district.
